周恩来病情持续恶化，癌细胞出现转移症状。中央政治局批准了吴阶平的医疗报告，决定对周恩来实施肿瘤切除手术。四届人大二次会议即将召开，有些问题必须要在会上解决，而且这一次领导班子组阁，周恩来怕一些人发难，导致下一步计划无法实施。周恩来向叶剑英建议推迟手术。吴阶平通过结肠镜检查的结果，又发现了新的肿瘤，经过会诊，总理的右侧结肠有多发性息肉，医疗组再次向中央打报告，要为周恩来进行手术。周恩来看着海棠花，不舍地离开了西花厅。中泰两国经过20年的努力终于建交，周恩来亲自出席签署仪式。蔡畅在理发店看到骨瘦如柴的周恩来后非常难过，连忙找到邓颖超询问周恩来的病情。然而中央有指示，邓颖超只好婉拒了蔡畅的问候。
